Appointment Hours
Black & Black Dental offers appointments during our regular business hours Monday through Thursday, 7:20 am to 5:40 pm.
Our office is closed Friday through Sunday, and on the following holidays: New Year's Day, Memorial Day, Observed July 4th holiday, Labor Day, Thanksgiving Day, Christmas Eve, and Christmas Day.
New Patient Forms
Thank you for choosing Black & Black Dental! New patients will be asked to complete a few important forms before their first visit at Black & Black. For your convenience, we have made these forms available below. Simply click the links below, print and complete the documents at a convenient time for you. Please bring the completed forms with you for your first visit. If you are unable to print at home, don't worry! We also have them available at our office for you to fill out in the waiting room before your appointment. Please arrive approximately fifteen minutes early so you can complete them before your appointment begins. We look forward to meeting you!
